Step 3: Ensure Correct Address and Vehicle Registration

Step 4: Mail Your Completed Application

Once your application is filled, mail it along with the required fee to the address specified on the form. Note that the CAV decals and ID cards can only be requested via mail and are usually processed within 30 business days. Unfortunately, DMV offices do not issue these items.

Remember, you must wait to drive in the HOV lanes as a sole occupant until you receive your decals and ID card, and affix the decals to your vehicle.
